DS-3053 — parental consent

  • Government-issued photo ID of the absent parent (passport, driver's license, or state ID — front and back)
  • Child's full legal name, date and place of birth
  • Other parent's full legal name and contact info
  • Photocopy of the absent parent's ID for the passport packet
  • Stable internet connection and a webcam for the video session
  • Quiet location — face must be clearly visible during the entire call
  • Signed-but-not-dated DS-3053 (we date it during notarization)

Florida apostille — what to bring

  • Original document or certified copy issued in Florida
  • Birth/marriage/death certificates: must be a certified copy from the Florida Bureau of Vital Statistics or county clerk (issued within the last 5 years preferred)
  • Notarized documents: notary block must include county, full name and current commission
  • Diplomas: signed by the school registrar, then notarized
  • Corporate documents: from the Florida Division of Corporations (sunbiz.org)
  • Federal documents (FBI background, IRS letter): originals only — go through US Dept. of State
  • Destination country name (some countries need additional translation)
  • Return shipping address inside the US
